Session 1: Access to Housing

Presenters Kristina Santana from NASTAD and Rita Harcrow from The National HIV/AIDS Housing Coalition will discuss how they developed the housing collaborative among select CDC-funded health departments and the impact that the collaborative had on partnerships and providing populations with access to housing.


Overview/Goal

The goal of the SDoH CoP is to build collaborative relationships among CDC-funded health departments, community-based organizations, and their partners implementing and evaluating plans to address SDoH. The SDoH CoP will also support the implementation and evaluation of these plans by sharing strategies, resources, and expertise to improve health outcomes among populations of focus. 

SMART Objectives

By March 31, 2027, participants of the SDoH CoP will be able to:

  • Identify and apply at least one actionable strategy to support the implementation of their SDoH plan, based on strategies, case studies, and peer learning shared during CoP sessions. 
  • Utilize at least one monitoring and evaluation tool or framework to assess the implementation and outcomes of their SDoH plan. 
  • Apply at least one insight gained from peer participants to strengthen the implementation or evaluation of their SDoH plan.
